During the time of the visit, the licensor observed that the provider occasionally transports children provider is missing a first aid kit, emergency contact information, and emergency authorization forms. The provider is missing emergency reflective triangles or another device to alert drivers in case of an emergency. Additionally, the provider does not have current insurance in place that covers the driver, the vehicle, and all occupants.
